Pharmacists can review medication in nursing homes

Pharmacists in Belgian nursing homes will be able to review residents’ medication plans, helping spot risky interactions, incorrect doses and prescriptions that may no longer be needed. The review will be coordinated with the treating or coordinating doctor and a nurse, while the doctor keeps final say on any changes after speaking with the resident or their representative. It will be free for residents and offered every two years, or sooner when prescribed, with priority for people with treatment-related concerns, psychotropic medicines, serious kidney problems or a recent hospital stay. The initiative aims to help older residents take only the medicines they truly need, and around 8,500 people are expected to benefit each year.
